We would ask patients, where possible, to come to the surgery between 09:30 and 17:00 to register to avoid the busy peak times at the start and end of the day.
(During the period of COVID-19 we ask patients to not attend the surgery for registration purposes, please email all completed registration documents together)
Your registration will be with the partnership. However, you have the right to express a preference for an individual GP. You may indicate this on the ‘application to join form’. However, please be aware that some of the GPs only work part time and not all of the GPs provide every service you may require. Your request, if made, will be entered onto the computer registration template as your preferred doctor.
Please complete a GMS1 form, Summary Care Record Factsheet, EPS Nomination Form and New Patient Questionnaire before coming to the surgery:

Proof of address will need to be attached via email with the forms, photo ID is also requested if you require Online Access
These forms are for pre-registration and convenience. You will still need to call in to the Surgery, either at 25 Mill Road or Staff Health Centre, to sign the forms and to show us your forms of identity.
Please try to obtain your NHS number before registering, as this will make the registration process much easier. You can find this on your NHS medical card, or alternatively your current GP will be able to give it to you. You will also need to bring proof of your address to the practice when you register such as a tenancy agreement or utility bill.
If you wish to register for the on-line facility, please provide originals of photographic identification along with a recent letter with identification of your name and full address for security purposes. Patients are to provide photocopies of these two documents along with the originals to scan into your records.

A Patient Guide to GP Services
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct. Please allow an extra 10 minutes for this process to be completed.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Changing Your Address
If you move to a new home, please inform us immediately, so we can update your records.
If you are out of our catchment area, you will be required to register with a GP Practice nearer your new home.
